    Mini Buffet Catering Order Form

    NOTICE:
    (1) Delivery charge of $30 per return trip is applicable.
    (2) Free set of disposable wares and serviette.
    (3) All Mini Buffet sets come in disposable aluminium trays or plastic containers.
    (4) Food best consumed within 1 hour.
    (5) Extra $1.00 per pax for menu change of Ayam Bakar Padang, Ayam Masak Merah, Ayam Rendang, Mutton Rendang, Ikan Masak Merah.
    (6) Order increment are to be done in multiples of 5. e.g. 30 pax to 35 pax
    (7) Delivery to non-permanent addresses (eg: chalet, resort, club, pub & etc.) will require pre-payment to be done via bank transfer.
    (8) Delivery time may differ up to 30 min due to traffic conditions.
    (9) A) Cancellation of catering order must be notified and reached to Rumah Makan Minang in writing/fax/email at least 3 working days before your function date. No phone cancellations are allowed unless as stated otherwise. B) The customer is liable to compensate Rumah Makan Minang 50% of the total catering costs should he/she cancels the order less than 3 working days from the function date. C) The customer is liable to compensate Rumah Makan Minang 100% of the total catering costs should he/she cancels the order less than 1 working day from the function date.
    (10) Preferred modes of payment are either cash upon delivery or cheque made payable to 'Rumah Makan Minang Pte Ltd'.
